Small Fleet DOT Compliance Checklist: What's Due and When

Every recurring DOT compliance task for owner-operators and small fleets, organized by when it comes due: hiring, monthly and quarterly deadlines, the yearly calendar and what happens if you miss one.

Small fleets answer to the same FMCSA rules as large carriers, usually without a safety department. This checklist from Diversified Safety & Compliance organizes the recurring work by when it comes due, so nothing catches you by surprise. It's a planning aid, not legal advice; the cited rules have the full requirements. Just starting out? Begin with the new trucking company checklist.

Every time you hire a driver

The calendar

  • January: check DOT's random testing rates for the new year; FMCSA's 2026 minimum is 50% for drugs and 10% for alcohol (49 CFR 382.305)
  • January 31: IFTA fourth-quarter return
  • April 30: IFTA first-quarter return
  • July 31: IFTA second-quarter return
  • August 31: Form 2290 for vehicles in use in July
  • October 31: IFTA third-quarter return
  • December 31: UCR for the coming year, and your IFTA license renewal
  • Your IRP renewal month: apportioned registration

Every 12 months, for every driver

  • An updated MVR and a documented review (49 CFR 391.25)
  • A limited Clearinghouse query (49 CFR 382.701)
  • Random testing throughout the year at the FMCSA minimum rates

Every 24 months: your MCS-150

Your MCS-150 update is due every two years, by the end of the month set by your USDOT number (49 CFR 390.19). The last digit sets the month: 1 is January, 2 is February, and so on through 9 for September, with 0 for October. The next-to-last digit sets the year: odd digits file in odd-numbered years, even digits in even-numbered years. The update now goes through FMCSA's Motus system.

On their own schedules

  • Medical certificates, which expire on the date the examiner sets. Book the DOT physical before the card runs out.
  • Supervisor training: anyone who makes reasonable suspicion calls needs 60 minutes on drugs and 60 minutes on alcohol before making one (49 CFR 382.603). See reasonable suspicion training.

When something happens

  • After a crash that meets the DOT criteria: alcohol testing within 8 hours and drug testing within 32 hours (49 CFR 382.303). See the post-accident testing checklist.
  • After a violation: removal from duty, the return-to-duty process with a substance abuse professional (SAP), then at least 6 directly observed follow-up tests in the first 12 months (49 CFR 40.307(d)).

What happens if you miss a deadline

  • MCS-150: FMCSA deactivates your USDOT number, and civil penalties can apply.
  • IFTA: in Nebraska, a late return draws a penalty of $50 or 10% of the net tax due, whichever is greater, plus interest.
  • Medical certificate: the driver isn't qualified to drive until a new one is issued.
  • Annual MVR review or Clearinghouse query: a gap an auditor will find, and a driver you can't show is qualified.

Small Fleet Compliance Questions

What does DOT compliance cost a small fleet each year?

With our 2027 owner-operator packages, from $228 a year for one driver with random testing and consortium membership, up to $1,287 a year with every filing, driver files and the Clearinghouse handled for 1 to 2 trucks. Tests other than randoms, DOT physicals and training are billed separately.

What happens if I miss my MCS-150 update?

FMCSA deactivates your USDOT number, and civil penalties can apply. The update is due every 24 months, in the month set by the last digit of your USDOT number (49 CFR 390.19).

What happens if I file IFTA late?

In Nebraska, a late return draws a penalty of $50 or 10% of the net tax due, whichever is greater, plus interest on any tax owed. The penalty applies even to a return with no tax due.

When is UCR due?

By December 31 for the coming registration year. The 2027 registration period opened October 1, 2026.

How often do drivers need an MVR and Clearinghouse query?

At least every 12 months for each driver: an updated MVR with a documented review (49 CFR 391.25) and a limited Clearinghouse query (49 CFR 382.701).
